A visit takes the dust and debris out of every supply and return line in the house and hauls it away, so none of it drifts back into your rooms.
The method matters more than the machine. Each register is covered, the collection hose goes onto the main trunk beside the air handler, and its suction keeps the ductwork under "negative air pressure" while an "air whip" works down each branch and knocks the buildup off the duct walls. The trade calls that "source removal": the debris travels out to the collection unit rather than back into a bedroom.

How to clean air ducts and vents?
Ducts are cleaned from the inside. The registers are covered, a collection vacuum on the main trunk holds the system under negative pressure, and an air whip or a "rotary brush" works down each line so the loosened debris is pulled out.
Wiping the grilles or reaching a shop vac a few inches into a register removes only the dust you can see.
What are the downsides to air duct cleaning?
The real downside is a poor job: a quick "blow and go" stirs the dust up without removing it and can leave the house dustier than before.
A full cleaning also means the system is off while the work goes on, with someone home to let us in. We quote the whole job before we start, so the cost is never the surprise.
